Four new laboratory-confirmed infections

South Africa has recorded four additional laboratory-confirmed mpox cases, taking the total reported since 1 January 2026 from 22 to 26, according to a Department of Health update released on Sunday, 18 October. The figures are cumulative for the year; they are not a count of people currently ill.

Where the cases have been recorded

The Western Cape accounts for 18 of the 26 cases. Gauteng has recorded three, the Eastern Cape and Free State two each, and KwaZulu-Natal one. Health officials say two Western Cape cases were imported and the other 16 were linked to local transmission. The department has not provided evidence that all four newly reported cases arose from the same chain of infection.

What the health authorities know

Of the 26 confirmed cases, 25 are male and one is female, with ages ranging from 23 to 50. Genetic clade information is available for 17 cases: 13 are linked to Clade Ib and four to Clade IIb. Those figures do not establish the clade of the nine cases for which sequencing information has not been reported.

Symptoms and the 21-day window

The department is urging people to seek healthcare if they develop a rash, fever, headache, muscle aches, swollen lymph nodes or fatigue. Anyone who has been in close contact with a suspected or confirmed case should tell a healthcare worker and monitor for symptoms for 21 days. If symptoms appear, the advice is to avoid close physical contact and seek assessment. People should also avoid sharing towels, clothing or bedding with anyone who may have mpox.

Why the update matters

The additional cases are a public-health surveillance development, not evidence of uncontrolled nationwide spread. Mpox can affect anyone exposed through close contact, and stigma can discourage timely testing. The next meaningful updates will be the locations and circumstances of new transmission, contact tracing results and any changes to the official health response.
